At Hollow Tree Kennel, we take great pride in raising healthy, intelligent, well socialized and “Happy” puppies with good
temperament. To help keep our puppies, and their parents healthy, we provide "NuVet Plus" vitamins daily, and also feed NutroMax
Puppy & Dog food. We sincerely hope you will enjoy your puppy for many years, and proper nutrition is the main ingredient to
having a healthy pet. Remember, all canine illness(es) are not prevented by vaccinations. Because of the variety & quality of dog
foods available, we require that all puppies purchased from Hollow Tree Kennel be kept on "NuVet Plus" Canine vitamins, and,
accordingly, offer a 2(two) year “guarantee of good health” for those puppies. This nutritional supplement is only available through
breeder recommendation and you are expected to have this on hand prior to receiving your puppy. The order line phone # is

a monthly report on which Hollow Tree Kennel puppies are being supplemented with "NuVet Plus" vitamins, and as such, allows us
to monitor the status of the “guarantee of good health". Accordingly, this “guarantee of good health” will be considered Null and Void
if you do not keep your Hollow Tree Kennel pet on this nutritional supplement.

If there is a congenital defect (not including hips, knees, umbilical or inguinal hernia, undescended testicle, under bite
or over bite) within 2 years from the date of birth, and it can be surgically corrected for less than the cost of a
replacement puppy, Hollow Tree Kennel will pay the actual cost of surgery (not the tests or exams prior to surgery)
up to the price you paid for your Hollow Tree Kennel puppy. If a condition arises that cannot be surgically corrected
and/or renders your Hollow Tree Kennel puppy’s health to a “terminal” status, please follow these procedures:

(1). Contact Hollow Tree Kennel immediately ;
(2). In counsel with your veterinarian and us, decide what/when must be done.

Provided your puppy/dog carries our kennel name in it’s Registered Name, has never been used for breeding, and
has not been sold or otherwise displaced from it’s original owner, and you certify that the puppy/dog has not been
over-exercised during it’s growth period (until 1.5 years of age) and that this Hollow Tree Kennel puppy/dog has
been given a "NuVet Plus" vitamin daily, since you received him/her, you may choose a replacement puppy, of equal
value, from any of our litters, subject to availability, paying the difference in price if you desire a more expensive
puppy. No cash refunds are given! This guarantee does not cover injury due to variances in environment and training
philosophies, and does not cover or include behavioral issues.

You, as the purchaser, must take your puppy to your veterinarian within the first 10 (ten) days after receiving the puppy and
keep your Hollow Tree Kennel puppy on a regular vaccination schedule and worming regimen by a licensed
veterinarian, and to follow his/her recommendation(s) regarding spay/neuter timing. All puppies will be purchased as
“Pet Quality” unless otherwise agreed to in writing at time of purchase, and therefore, will be required to be spayed
or neutered prior to 6 months of age. Please initial the appropriate space below:

Pet Quality: It is understood that this puppy must be spayed / neutered by six months of age and WILL NOT
be used for breeding. Purchaser agrees to provide a copy of the spay/neuter certificate to

Hollow Tree Kennel guarantees this puppy to be free of all communicable disease for a period of 10 days from the
date of this agreement, and also guarantee same puppy to be free of genetic defects which could make this puppy
not suitable for the purpose of a “Family Pet” until the puppy attains the age of two (2) years provided all
previously named conditions/provisions stated herein are adhered to. Purchaser(s) understand and agree that the
replacement option is valid only when health problems are hereditary and not due to contagious disease or other
illness and that this “guarantee of good health” will expire on _______________(date).
Should this puppy display any indication of poor health, you must notify Hollow Tree Kennel at (417) 858-6033, or e-mail
@ hollowtreekennel@centurytel.net, within 10 days to make a claim for a replacement puppy. If your puppy is found
to be very ill or has a congenital condition covered within the “guarantee of good health” , obtain a letter from your
veterinarian on his/her office stationary attesting to diagnosis and relative symptoms, return this letter, along with the puppy’s
registration papers and the puppy to Hollow Tree Kennel, within 72 hours, and, if you qualify for a replacement, you
can choose a new puppy of equal value. If you select a puppy of greater value, you will be responsible to pay the
difference. If we do not have a puppy of equal value available within 180 days, we will refund your puppies original
purchase of $__________, minus Delivery fees of $_____________.

If, for any reason, you decide to surrender this puppy, you must notify HollowTree Kennel. It is understood and ageed to
that this puppy will NOT be surrendered to an animal shelter, humane society facility, or rescue Organization.

This agreement is a legal and binding contract entered into without coercion, between the purchaser(s) named herin and HollowTree
Kennel, in the state of Missouri, and is the entire agreement between buyer and seller.
